WAIVER OF LIABILITY AND RELEASE

This form must be acknowledged by the player's parent or legal guardian for each flag football participant. Without acknowledging this form, no player will be allowed to participate in Sports 4 Life Athletics NFL Flag League games or events.

I, the parent or legal guardian, in consideration for my child’s voluntary participation in organized Sports 4 Life Athletics NFL Flag League activities, do hereby willfully acknowledge that by checking the box during registration, I am confirming my understanding and agreement to the following:

• My child’s player status will be kept in good standing. I will ensure they conduct themselves in a manner that reflects positively on the league. I understand that players may be dismissed from participation, without refund, for violent conduct, unsportsmanlike behavior, or actions deemed harmful to the league on or off the field. I agree to pay for all damages to property caused by my child willfully, negligently, or otherwise.
  

• Flag football is a physical sport that involves the risk of injury. I assume all risks and hazards associated with my child’s participation in Sports 4 Life Athletics NFL Flag League. I affirm that my child is in appropriate physical condition to participate in practices and games, and has no illness, condition, or injury that would be aggravated by their involvement. If this status changes, I will notify the league or coach immediately. I understand that participation carries the risk of serious injury or death, including potential complications from the unavailability of immediate emergency medical care. I confirm a current medical consent form is on file and that my child will wear properly-fitted and appropriate gear, including a mouthpiece, per league rules.

• Photo & Media Consent: I authorize the use of my child’s photograph, image, likeness, and/or voice to appear in any league- or event-related media, including promotional materials, website content, social media, television, video, or radio coverage, without compensation.

• I hereby release, waive liability, discharge, hold harmless, indemnify, and covenant not to sue Sports 4 Life Athletics NFL Flag League, its directors, officers, coaches, employees, volunteers, sponsors, advertisers, agents, and affiliated organizations. This release extends to field owners, facility providers, municipal or government agencies, and any successors, heirs, or assigns.

I have completely read and fully understand this document. I acknowledge that by agreeing to it during registration, I am giving up substantial legal rights on behalf of myself and my child. I do so voluntarily and of my own free will.

As the parent and natural or legal guardian of the participant (minor), I confirm that I have the legal capacity and authority to execute this waiver on behalf of the minor.
